In consideration of participating in the SPORT OF PAINTBALL I represent, that I understand the nature of this Activity and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participating in the Activity. I fully understand that this Activity involve risks of serious bodily injury, Including permanent disability, paralysis and or death.

I understand that removing a mask in the playing area, either in front of or behind the netted area, or removing a barrel sock in staging area, or the violation of any of the other safety rules outlined in briefings or by the employees could be potential dangerous to myself and others and is grounds for immediate suspension of playing privileges without any refund.

I further understand that as a public facility, pictures and or video may be taken by management staff or others, and I grant them the right to modify, publish, and re-publish video, photographic portraits or pictures of me in which I may be included, in whole or in part.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, AND I FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I HAVE GIVEN UP LEGAL RIGHTS BY SIGNING IT AND SUBMITTING THIS FORM, AND I AM SIGNING IT AND SUBMITTING THIS FORM FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
